Braces cost and payment plans: what you should know

Educational article · 7 min read · Updated 2026

Dental model and a calculator on a clinic desk

We are a family dental clinic in Taman Permata, Kuala Lumpur, a short drive from Wangsa Maju, Setapak, Melawati and Keramat. At our clinic, metal braces are RM6,000 all-inclusive. Braces may not suit everyone; suitability is confirmed after an examination.

Cost is the first thing most people ask about braces, and fair enough. It is a real amount of money spread over a long stretch of time. This article explains what actually drives the price, what an all-inclusive fee should cover, and the questions worth asking at any clinic before you commit.

What actually drives the cost

Two people can walk into the same clinic and be quoted different amounts. That is normal, and here is why.

How complex the case is

Mildly crooked front teeth and a badly crowded bite are not the same job. More movement means more visits, more wire changes and a longer treatment time.

The type of braces

Metal brackets are usually the most affordable. Self-ligating, ceramic, lingual and gold brackets cost more because of the materials and the extra chair time they need.

Preparation before fitting

Some people need scaling, fillings or extractions before the brackets can go on. Here, the scaling and any extractions the treatment plan needs are already inside the braces price.

A quote given over the phone or over chat is an estimate, not a price. Nobody can size up your case properly without looking in your mouth and, usually, taking an X-ray first.

What “all-inclusive” should mean

Braces are not a single purchase. They are a treatment that runs for many months, and small charges can appear all the way through it. An all-inclusive fee rolls those charges into one number at the start, so the figure you are quoted is the figure you end up paying.

The word is not regulated, though. Two clinics can both say “all-inclusive” and mean different things. So the useful question is not whether a payment plan is all-inclusive, it is what sits inside it.

Items a payment plan can cover

  • ✓Retainer at the end. Teeth drift after the brackets come off. The retainer is what holds the result, so it is part of the treatment, not an optional add-on.
  • ✓Extractions, if your case needs them. Crowded arches sometimes need space made before the teeth can be aligned.
  • ✓Scaling. Cleaning before the brackets go on, and again when they come off.
  • ✓Rebonding. Brackets do come loose, usually from hard or sticky food. Ask whether rebonding is charged each time.
  • ✓Adjustment visits. The routine appointments where wires are changed and progress is checked.

Things people forget to ask about

  • ?X-rays and records. Are the initial X-ray, photos and study models inside the quoted price, or billed separately?
  • ?Removal of the braces. Taking the brackets off is a separate procedure at some clinics. Check whether it is inside the payment plan.
  • ?A replacement retainer. Retainers get lost or stepped on. A first retainer being included does not always mean a second one is.
  • ?Missed appointments. Is there a charge if you cannot make a visit, and how much notice is needed?
  • ?If treatment runs long. If the case takes more months than first estimated, does the fee change?

Five questions to ask any clinic

Take these to your examination. A clinic that is comfortable with its pricing will answer all five without hesitating, and will put the answers in writing.

1 “What is the total, from today until the braces come off?” One number, not a monthly figure. Monthly amounts are easy to compare and easy to misread.
2 “What is not included in that number?” Better than asking what is included. It brings the exceptions out into the open.
3 “Are extractions, scaling, X-rays and the retainer inside the price?” These four are the usual source of a bill that grows after you start.
4 “If a bracket comes off, is rebonding charged?” Over 18 to 24 months this happens to most people at least once.
5 “Can I have the payment schedule in writing?” Deposit, monthly amounts, how many months, and what is left at the end. On paper, before you pay anything.

How instalment plans work

Because braces run over a long period, most clinics let you pay in stages rather than all at once. The usual shape is a deposit when the brackets go on, a short run of larger monthly payments, then smaller amounts paid at each visit while treatment continues.

Two things matter more than the monthly figure. First, whether the plan is interest free, or whether the total climbs the longer you take. Second, when the remaining balance is due. Some plans expect the balance to be settled before the braces are removed, so it is worth knowing that on day one rather than in month twenty.

An example: how our own two plans are structured

We publish ours so you can see the shape of a plan clearly. Both payment plans come to the same total of RM6,000 all-inclusive. The only difference is how the early months are spread.

Both payment plans settle any remaining balance when the braces come off. Full details sit on the pricing page.

Why bundling changes the maths

It helps to see what the bundled items cost on their own. These are our published fees for the same procedures when they are done outside a braces fee. Add two extractions, a scaling before the braces go on and the removal appointment (which includes the second scaling), and you can see how much the RM6,000 already covers.

Our published fees for these procedures when carried out on their own, outside a braces fee.
Procedure Fee on its own
Scaling, adultRM150
Extraction, permanent toothRM150
Removal of braces, scaling includedRM500
Retention pinRM30

In our payment plans these sit inside the RM6,000, along with the retainer and rebonding if a bracket comes off. That is what we mean when we say all-inclusive.

Planning the money side

  • Work out the monthly figure honestly. Take the total, subtract the deposit, and divide by the number of months you expect to be in treatment. If that number is uncomfortable now, it will still be uncomfortable in month ten.
  • Leave room for the ordinary stuff. Toothbrushes for braces, interdental brushes and the occasional replacement wax are small, but they are real and they recur.
  • Check whether your insurance or company panel covers any of it. Orthodontic work is often excluded, but the check-up, scaling or an extraction sometimes is not. Ask before you assume either way.
  • Do not stretch to the most expensive bracket type by default. If you want clear or hidden braces, ceramic or lingual brackets are worth the difference. If not, metal does the same job on the teeth.
  • Ask what happens if you need to pause. Life happens. Knowing the clinic's position on a missed month before you need it is better than finding out afterwards.

Our honest advice

Do not choose dental care on price alone. You will be sitting in that chair every four to six weeks for a year and a half or more, so being comfortable with the dentist and understanding the plan matter just as much as the number.

Get the examination done, get the quote in writing, read what is inside it, and ask about anything that is not. A clinic that welcomes those questions is usually a clinic that is straight with you about everything else too.
